Are you interested in playing sports at Novato High School?
All athletes must register, submit a physical form signed by a doctor, and take a concussion test every year. No student-athlete may participate with any team without registering prior to the first day of practice, submitting a physical form, and taking a concussion test.
Step 1
Register for all sports you intend to
play for the year.
MUST have a physical form signed before registration is completed.
Step 2
Complete the Physician’s Clearance Form dated AFTER MAY 2025.
Must have form signed to complete registration
Step 3
Register to participate in sports by going to Step 1 and then taking an impact concussion test. Required once per academic year. NO SIGN-UP NECESSARY, but you must complete Step #1 in order to take the test. An email will be sent to you in July with dates to take the test.
